This form may be used to place an order with KoaFurniture.com. Please fill out the form and submit by email or via postal mail to the address on the order form. You also may call and speak with me directly and I will take your order over the phone.
Please confirm availability of selected wood type before ordering.
All orders require a 50% deposit when making order.
